Mr A complained that Hywel Dda University Health Board failed to respond to the complaint he submitted in July 2025.

The Ombudsman found that the Health Board failed to provide Mr A with a final complaint response. The Ombudsman said this caused uncertainty and frustration for Mr A and decided to settle the complaint without an investigation.

The Ombudsman sought and gained the Health Board’s agreement to, within 6 weeks, provide a final complaint response, apologise for the delay and explain why this happened, and offer a £100 financial redress payment in recognition of the delays.
