Ms A complained that Betsi Cadwaladr University Health Board declined to complete an ADHD assessment, as based on her completed self-assessment form she did not meet the criteria for a formal assessment. Ms A said that she had never received or completed a self-assessment form.

The Ombudsman found that the Health Board’s assessment of Ms A was based on her typed notes that had been sent with the GP’s initial referral rather than the self-assessment form referred to by the Health Board in its response to Ms A’s complaint.

The Ombudsman contacted the Health Board and in resolution of Ms A’s complaint it agreed to, within 20 working days, send the self-assessment form for Ms A to complete and apologise for the failure to send this previously. Within a further 30 working days, consider whether Ms A met the criteria for an ADHD assessment based on the self-assessment form. The Ombudsman considered this to be an appropriate resolution and did not investigate.
