Mr B complained that his father, Mr A, was discharged prematurely by Cwm Taf Morgannwg University Health Board (“the Health Board”) from a hospital on 20 August 2024, without a care plan or appropriate assessment of risk.

The investigation found that Mr A’s discharge from the hospital was appropriate and so the complaint was not upheld. However, the Health Board was invited to provide out-of-hours contact details to patients who have been fitted with a stent and discharged on the same day.
