Ms A complained that the Health Board had failed to fully investigate and respond to her concerns about the care and treatment provided to her late sister.

The Ombudsman was concerned that whilst the Health Board had responded to some of Ms A’s concerns, it had failed to investigate and respond to other significant concerns she had raised when she obtained a copy of her late sister’s medical records and the Medical Examiner’s report.

The Ombudsman sought and obtained the Health Board’s agreement to investigate and respond to those concerns within 3 months.
