Ms X complained to the Health Board about comments made by a psychologist which she discovered in her medical records. She complained to the Ombudsman that the Health Board had failed to provide its formal response to her complaint.

The Ombudsman noted that the Health Board had told Ms X that although the investigation into the concerns raised had been completed, the matter had been passed to the Legal & Risk Team for consideration before being issued to the complainant. The Ombudsman made enquiries with the Health Board and accepted the explanation as to why a formal response had not yet been issued.

The Ombudsman sought and gained the Health Board’s agreement to provide the formal written response to Ms X by 14 November 2022.
