Mr X complained to Betsi Cadwaladr University Health Board (“the Health Board”) about the care and treatment provided to his wife. The Health Board’s investigation included a review of the oncology notes and speaking to staff involved in his wife’s care. However, it did not have access to his wife’s medical records prior to preparing its response. Mr X complained to the Ombudsman that he was unhappy with the Health Board’s response.

Following discussion with the Ombudsman, the Health Board confirmed that it had subsequently located the medical records. The Health Board agreed to provide a further written response to Mr X within 30 working days of the Ombudsman’s decision.
