Summary (the LGO's words, from its website)
The Council failed to arrange alternative provision and secure the provision in Mr X’s child, Y’s, Education, Health and Care (EHC) Plan. This caused Y to miss two and a half terms of provision. The Council also delayed issuing a final amended EHC Plan following Y’s annual review. The Council has agreed to apologise, make a payment to Mr X for the impact on Y, and review its processes.
