We found fault by the Council and Trust with regards to how they handled a child protection medical examination for Miss B. This caused Miss B and her mother, Mrs X, distress and confusion. The Council and Trust will apologise and take action to prevent similar problems occurring in future. They will also pay Miss B and Mrs X a symbolic remedy in recognition of the injustice caused to them.
