Recommendations
The appropriate authorities should ensure that common abbreviations and phrases are used to denote observations and symptoms and the tests carried out to check for or eliminate certain conditions are standardised. In this way others requiring to access notes can be confident of what they mean and the steps carried out at the time when the patient was seen. Guidelines should be promoted whereby the doctor in charge of ward round checks, countersigns and if necessary amends the case notes made of the ward round by a Junior Doctor.
Under section 28 of the 2016 Act, each recipient of a recommendation must respond within 8 weeks.
