⚠ Chemotherapy was stored in the treatment room. We observed the door was left open throughout the day, which provided opportunities for unauthorised access to this room and meant chemotherapy medicines were not secure. Ensure that patients and staff are able to use emergency call bells in all toilet cubicles.

⚠ Of three patient toilets, we found one did not have a call bell to summon help in an emergency. This meant that should a patient become unwell in this area their safety could be compromised as they would be unable to summon staff assistance.
