Mrs A complained about her late mother’s care and management, when she presented to the Emergency Department on 16 February 2022 and her subsequent death. Mrs A said that the Health Board did not apologise to her for the failings in her mother’s care as part of its Serious Incident Review (“SIR”) and she did not feel that lessons had been learnt or that measures had been put in place to prevent similar occurrences.

The Health Board agreed to provide an apology to Mrs A and her family for the shortcomings in the management and care of their late mother as well as providing a full explanation of the measures introduced as a result. In addition, the Health Board agreed to apologise to the family for shortcomings around its complaint processes when it came to complaint handling.
