Full decision

Miss M complained about the delay in the diagnosis of ovarian cancer, and failings in Hywel Dda University Health Board (“the HealthBoard’s”) communication with her.

The Health Board agreed to instruct an independent clinical expert, with input from Miss M, to review the management of her care. It also agreed to consider any failings identified for redress (in a process analogous to that, contained in the complaints process), and to act upon any learning or recommendations made in respect of Miss M’s treatment or for future patients. The Health Board agreed to draft the letter of instruction for Miss M’s agreement within 10 working days, and indicated that the process was likely to take approximately 3 months.
