Mr X’s GP referred him to Ysbyty Glan Clwyd on 29 November 2019 and 26 February 2020 on an urgent basis with pilonidal sinus symptoms. Mr X attended the Hospital’s ED on 22 June, a MRI scan was requested, but Mr X left before he was discharged. Mr X attended the ED on 25 June but left without being seen. On 16 July Mr X had an MRI scan that found no evidence of a focal collection, significant inflammation, fistula or abnormality of the pelvis. On 13 August Mr X attended the ED, he left and signed a form that he had chosen not to wait for a consultation against nursing advice. Mr X sought private treatment and on 23 October Mr X’s pilonidal sinus was excised. Mr X complained whether the 16 July MRI scan was correctly reported and the effect this had on his subsequent treatment.The Ombudsman found when Mr X left the ED on 25 June and 13 August the outcomes could not be assumed as he had not waited to be seen. He found that the 16 July scan was appropriately and correctly reported, and it had not affected his subsequent treatment. The Ombudsman did not uphold the complaint.
