Summary (the LGO's words, from its website)
There was fault by the Council. The Council failed to issue a new final Education, Health and Care Plan after annual reviews in 2024 and 2025. A child, Y, received none of the educational provision in their Education, Health and Care plan for five terms. The family were not able to appeal to the SEND tribunal as they did not have an updated plan to appeal. A symbolic payment and review of procedures remedies the injustice to Y.
