We found fault with Lewisham and Greenwich NHS Trust’s handling of Mr C’s discharge from hospital in September 2023. This fault caused Mr C’s daughter, Mrs B, confusion and distress. The Trust will apologise to Mrs B and pay her a financial remedy. It will also take action to prevent similar problems occurring in future. We found no fault with the actions of London Borough of Bexley in the discharge process.
