Mrs X complained the Council failed to provide support for her son, Mr Y, and failed to reimburse her costs for Mr Y’s support. She also complained it delayed completing an annual review of Mr Y’s Education, Health and Care Plan. We found the Council at fault for failing to complete an annual review of Mr Y’s Education, Health and Care Plan and failing to provide him with support it said it would. This fault caused injustice to Mrs X and Mr Y. The Council agreed to apologise, review Mr Y’s Education, Health and Care Plan, and reimburse Mrs X's support costs to remedy injustice.
