That being said, I think it is important that there should be full dialogue between the lead professional in the patient’s care in the hospital and the care home manager before any patient is transferred from hospital to a care home. This dialogue should include the patient and, if possible, a family member and should be fully documented.
Under section 28 of the 2016 Act, each recipient of a recommendation must respond within 8 weeks.
