⚠ Patients' bedrooms did not have locks, preventing them from securing their rooms and protecting their possessions. This resulted in property going missing from their rooms and affected their privacy and dignity.

⚠ Staff were unclear on the trust’s child visitor’s policy and where patients and child visitors could meet on the ward. They did not follow the policy for pre-arranged visits, and one child visitor became upset during a family visit due to another patient accessing the room.
