The trust must ensure patient records include all required information about the patient.

⚠ Patient records did not include all required information such as patient observations. We found that in one of the three patient records an early warning score was inaccurate due to missing observations, and in another, the early warning score was not recorded.

The trust must ensure there is an appropriate record of all medicines administered.

⚠ Not all records of medicine administration were up to date and accurate. Midwives had not consistently prescribed medicines given in labours, such as Entonox, on treatment charts, which is against medicines legislation and NMC practice standards.

The trust must review the risks relating to the environment of the MLU to ensure it is fit for the purpose of providing a homely environment for low risk women to give birth.
