The registered person had systems or processes in place that operated ineffectively in that they failed to enable the registered person to assess, monitor and improve the quality and safety of the services being provided. In particular: • Systems and processes failed to ensure all staff were inducted and trained suitably and effectively to mitigate safety risks for patients. For example, basic life support, infection prevention and equality and diversity training. • Policies and procedures were not always consistent or clear for staff to follow. • Recruitment checks had not been completed in full in line with service policies or schedule 3 guidance. This was a breach of Regulation 17(2)(b) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.
