⚠ Goodgovernancesystemsandprocesseswerenotalways effectiveorfollowed.Inparticular: Therewasalackofoversighttoensurepoliciesand proceduresregardingmedicinesmanagementand effectivemanagementofpatientswasapplied. Safetyalertswerenotalwaysfollowed.Forexample, • Therewere39femalepatientsofchild-bearingagewho wereprescribedmedicinesthatmaycauseside-effects inpregnancy.Wereviewed5ofthesepatientsand found3hadeffectivecontraceptioninplace.Therewas nodocumentationtoshowanyofthepatientshad beeninformedoftheriskofthesemedicinesin pregnancy. • Wefound50patientswereprescribedaspecific medicinetotreatdiabetesthathadararebutserious sideeffect.Asafetyalertissuedin2019,advised clinicianstoensurepatientssoughturgentmedical attentioniftheyexperiencedsymptoms.Asampleof5 ofthe50recordswerereviewedandtherewasno evidencetosuggestpatientshadbeeninformed. Areviewofthepatientrecordsystemshowedthepractice hadcarriedout261medicinereviewsintheprevious12 months.Wereviewed5patientsandfounddocumentation wasnotfullydetailed.Forexample,2patientswere prescribedhighstrengthpainreliefmedicines.Itwas notedthatcompliancewaschecked.However,therewas notarecordofwhetherthemedicineswereeffective.Two ofthereviewshadnotconsideredallthemedicines prescribedtothepatients. Themonitoringofpatientswithpoorlycontrolleddiabetes wasnotconsistent.ThiswasinbreachofRegulation17(1)oftheHealthand SocialCareAct2008(RegulatedActivities)Regulations 2014.
