Mr X complained that Hywel Dda University Health Board had failed to provide a further complaint response to address the additional concerns raised during a meeting in October 2023.

The Ombudsman decided that there had been delays and oversights with the response, which she said had caused uncertainty, frustration, and inconvenience to Mr X. She decided to settle the complaint without an investigation.

The Ombudsman sought and gained the Health Board’s agreement to issue its further complaint response and offer an apology for the delay and inconvenience caused within 30 working days.
