Mr S complained that Hywel Dda University Health Board failed to investigate the circumstances of his father’s passing in February 2022.

The Ombudsman found that although the Health Board had issued an initial response in October 2022, further questions were raised that required a response. She said that there had been a significant delay in issuing Mr S with his further response, and that it had failed to provide meaningful updates. She said this caused frustration to Mr S.

She decided to settle the complaint without an investigation.

The Ombudsman sought and gained the Health Boards agreement to provide Mr S with an apology and explanation for the delays and issue any outstanding responses within 6 weeks.
