Mr T complained that Cardiff and Vale University Health Board failed to issue a written response explaining its findings and views to concerns she raised about the care and treatment provided to her late father.

The Ombudsman found that the Health Board met with Mr T on two occasions to discuss the investigations but failed to follow up in writing. She said that this caused additional frustration and inconvenience to Mr T.

The Ombudsman decided to settlement the complaint without an investigation and sought and gained the Health Board’s agreement to apologise for not providing a written response and to issue its response within 30 working days.
