The Prisons and Probation Ombudsman aims to make a significant contribution to safer, fairer custody and community supervision. One of the most important ways in which we work towards that aim is by carrying out independent investigations into deaths, due to any cause, of prisoners, young people in detention, residents of approved premises and detainees in immigration centres. 2. If my office is to best assist His Majesty’s Prison and Probation Service (HMPPS) in ensuring the standard of care received by those within service remit is appropriate, our recommendations should be focused, evidenced and viable. This is especially the case if there is evidence of systemic failure. 3. In 2010, Mr Graham Glennie was sentenced to an imprisonment for public protection (IPP) for sexual offences, with a minimum time he would spend in prison of nearly three and a half years. He was released in March 2022. In January 2023, Mr Glennie was recalled to prison. In August, he transferred to HMP Whatton, and in November he was sentenced to 20 weeks imprisonment for failing to comply with police requirements. 4. Mr Glennie died of metastatic cancer of unknown primary (when cancer spreads around the body, but it is not possible to identify the original tumour location) on 7 December 2024, in hospital. He was 65 years old. We offer our condolences to those who knew Mr Glennie. 5. NHS England commissioned an independent clinical reviewer to review Mr Glennie’s clinical care at Whatton. 6. The clinical reviewer concluded that the clinical care Mr Glennie received at Whatton was equivalent to that which he could have expected to receive in the community. She found that Mr Glennie’s medical records contained evidence of good, individualised management of long-term conditions and end of life care. 7. The PPO investigator investigated the non-clinical issues relating to Mr Glennie’s care. We did not find any non-clinical issues of concern. 8. We make no recommendations. 9. The initial report was shared with HM Prison and Probation Service (HMPPS). HMPPS did not find any factual inaccuracies.
