Patient Y was convicted of the murder of Mr Z in June 2011. The patient used services provided by South Essex Partnership University NHS Foundation Trust. Services have now transferred to East London NHS Foundation Trust.

Action Required: NHS England Regional Teams to further develop resources for families. Identified Lead: NHS England Regional Independent Investigation Leads. Target Date: March 2016. Progress: Work is in progress to develop further resources for families. ‘Making Families Count’ videos have been completed and are available on Vimeo for all to use. Partnership working with Hundred Families, advocacy for families affected by homicides continues. Leaflet for families with contact details for the regional teams on their website. NHS Leaflet/electronic guidance for families to be developed.
