This is the report of the independent investigation into the care and treatment of Miss E . At the time of the homicide (2011) Miss E was receiving mental health services provided by Lancashire Care NHS Foundation Trust. The associated action plan has been published by the trust.

Recommendation
The trust should ensure that all calls to any out-of-hours phone services and helplines are risk assessed to see whether any immediate action needs to take place.

The majority of ccommunity mental health seervices within thee Trust are provided between the hours of nine to five Monday to Friday. Those seervices that are provided seven days a week and over an exteended day are provided by coree staff. The Crisis Reesolution and Homme Treatment Teams and Helpline operationnal protocols will be updated too ensure that all ccalls are approved. recorded and include guidancee on: - Risk Assesssment of telephonne calls - Recording aand passing on innformation - Arrangemennts for joint workinng and information shharing between thhe two services

The Crisis Resoolution and Homee Treatment Teamms and the Mental Health HHelpline provide telephone accesss out-of hours. Both services haave operation protocols in placce to support practice. To communiccate and implemeent the changes required. To undertake an audit of comppliance to ensuree practice has bbeen embedded.

Recommendation
The trust and the GP practice must ensure that all clinical staff carry out their responsibilities in line with local adult safeguarding policy and good practice guidance.

The Trust has roobust Governance proocesses to support Adult Saafeguarding. Progress againsst the Trust Safeguarding Sttrategy is reported to the SSafeguarding Committee and to the Trust Quality Committtee. The report to be presented to Trust and Network governance forums Daring to Shaare Events to be hheld across the Trust to commmunicate the learrning from the investigation. Adult Safeguaarding Audit to bee undertaken of GP and Trustt practice. Results and aactions from the aaudit to be shared with CCommissioners annd NHS Englandd.
