Recommendation
The Trust should issue a clinical update reminding staff that drug use in young people is fairly common. It should be a matter of routine to consider carefully the role of illicit drug use in a patient's presentation.

The Trust should amend its incident reporting policy to make it clear that all cases that fall within the criteria of NHD 1. (170) will automatically be the subject of a 60-day incident (GIR) review following an initial reflective review.
