In discussion with the trust we have identified a number of key themes arising from this investigation. We recommend that these themes should be part of a short independent review but carried out in partnership with the trust. The objective of the review would be to identify how effectively the themes have been embedded within the new team structures and services and what, if any, further action needs to be taken. The trust is supportive of this approach.
