This should include consideration of any support that the said Care Coordinator should have received in order to ascertain whether there were any systems failures that prevented the individual from practising in accordance with both national and local policy and procedure. The Trust should also review all of the Care Coordinator’s previous caseload to assure the Trust Board that there are no other cases that require urgent remedial action to be taken. The Trust will be expected to act in accordance with its internal policies and procedures in determining any further actions that need to be taken.
