The IOPC recommends that Derbyshire Constabulary should implement improvements in longer term safeguarding practices within high-risk investigations. This recommendation was raised by the investigating officer as the result of a death or serious injury investigation. Suspects who qualify for a 'Fit to Release' assessment prior to release from custody, should be robustly safeguarded throughout the investigation by the following; - The Suicide Risk Management Plan must be updated following every interaction with the suspect. - The suspect should be spoken to (ideally in person) to facilitate the re-bail process. - The suspect should be asked relevant questions regarding their welfare at every interaction and an Adult at Risk PNN should be submitted if necessary. - Supervisors should ensure that the suspect's welfare is considered and documented appropriately. These changes will ensure that suspects are appropriately safeguarded and may help to prevent suspects taking their own lives.

Accepted The recommendations are to be published internally on Chief Constable’s Orders. Additionally, a support booklet for suspects investigated for PCOT offences is in the process of being compiled, this will aid signposting and reduce suicide risks.
