HSSIB has modelled an approach to investigating patient safety events under the NHS Patient Safety Incident Response Framework (PSIRF). This report considers the care of a person with a mental health condition who was being cared for on a mental health low secure unit. His physical health deteriorated and he had to be admitted to an emergency department.
