PN/18/02/2026/SLI 315698488 I hereby give you notice that I am of the opinion that the following activities namely: All works in the excavation at Great Horton Service Station, 472 Great Horton Road, Bradford BD7 3HR other than to make safe under the instruction of a competent engineer (civil or structural) involve a risk of serious personal injury, and that the matters which give rise to the said risks are: the uncontrolled collapse of the excavation face as work equipment is being used on top of the excavation face without suitable means to prevent the face collapsing because suitable + sufficient steps have not been taken to precent any person, work equipment or any accumulation of material from falling into any excavation. This being a matter within your control.

PN PT13/06/2026-01 -I hereby give you notice that I am of the following opinion that the following activities namely all works removing the asbestos cement not sheets at the premises save for safe remedial measures. People are liable to be exposed to and inhale asbestos dusts arising for work activity. this may cause significant and serious long term health effects. You are carrying out work that has disturbed asbestos contaminating materials - asbestos sheeting that are located on the former chicken shed, structures that are liable to give rise to asbestos dusts and you have not taken suitable steps to prevent exposure and spread of contamination around the site. INPT16/01/2026-02 315592412 You have failed to undertake suitable non-notifiable asbestos removal training, or provide this to your employees, prior to undertaking non-notifiable asbestos removal work. INPT16/01/2026-01 315592417 Waste which contains asbestos should not be stored, except in a sealed receptacle such as a lockable, totally enclosed skip or container. You failed to ensure that appropriate measures were in place to comply with this statutory provision. INPT16/01/2026-03 315592420 You have failed to ensure that personal protective equipment, namely respiratory protective equipment, provided in accordance with this regulation is suitable for its purpose, i.e. the operatives have not been face fit tested for use of the tight-fitting RPE INPT16/01/2026-04 315592429 You do not have suitable arrangements in place to plan, manage, monitor or coordinate construction work carried out by you or workers under your control, to ensure that, so far as is reasonably practicable, construction work is carried out without risks to health or safety, as demonstrated by the inadequately controlled risks during demolition of the former poultry sheds.
