Summary
The review identified systemic failures in agencies' recognition and response to the victim's disclosures of emotional and economic abuse, including coercive control, and the link to suicide risk. There were missed opportunities for comprehensive risk assessments, multi-agency information sharing, and timely support, particularly when the victim returned to the perpetrator's home.
