Summary

This DHR identifies systemic failures in identifying the true level of risk posed by the perpetrator's long-term, controlling, and coercive domestic abuse. Agencies missed opportunities to intervene effectively, partly due to the victim's minimisation and challenges in information sharing, despite the victim's death being attributed to cocaine toxicity following a severe assault.
