Summary
The review found no clear indicators that could have prevented the homicide, noting limited agency contact and no significant concerns. However, it identified missed opportunities for the school to address the perpetrator's hearing aid use and for the GP to probe the victim's social situation. A new vulnerability policy for the Council was also developed.
