Agencies failed to consistently recognise and respond to domestic abuse, especially in interfamilial relationships and when alcohol misuse was a factor. Challenges were noted in mental health and alcohol care pathways for intoxicated individuals, missed safeguarding opportunities, and managing historical abuse allegations.
