1. By 1 November 2025, people must be supported to experience care and support that is safe and right for them. To do this the provider must ensure that: a. Records used to evaluate people’s health and wellbeing are accurately completed. This should include but is not limited to oral intake records. This information must be used to evaluate the effectiveness of interventions at regular intervals throughout the day and direct staff on how to support people. This is to comply with Regulation 4(1)(a) and Regulation 5(b) (i)(ii) and (iii) of The Social Care and Social Work Improvement Scotland (Requirements for Care Services) Regulations 2011. This is to ensure that people's health benefits from their care and support and takes account of the Health and Social Care Standards (HSCS) which state: "My future care and support needs are anticipated as part of my assessment" (HSCS 1.14) and "My personal plan (sometimes referred to as a care plan) is right for me because it sets ou

This must include, but is not limited to ensuring that: a) staff complete and record an accurate assessment of individuals’ health, physical and mental health needs. This must include, but is not limited to - risk of falls, nutritional needs, continence/skin care needs, communication and preferences of medication b) personal plans are implemented, and care is delivered in accordance with the assessed needs of each individual service user c) improved monitoring and review systems are implemented to evaluate the effectiveness of care interventions and the outcomes used to direct staff on how to support people.
