1. To ensure that care and support continues to meet people's needs the provider should formally review individual care plans and risk assessments and make certain that outcomes from these inform individual care plans. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which states that: 1.19 'My care and support meets my needs and is right for me' and 2.17 'I am fully involved in developing and reviewing my personal plan, which is always available to me.'
Conditions of registration
The service will be provided to adults and older people with learning and/or physical disability living in their own homes and in the community.
Where the support is provided from more than one address the service must keep the Care Inspectorate informed of any changes to the addresses from which the service is provided.
The service will be provided by one staff team supporting Falkirk Stirling and Fife.
The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details as it appears on this certificate.
The registered manager will also be the registered manager of Thistle Care Solutions support service care at home CS2020380086.
