Recommendation 1

1. The provider should further develop support planning to ensure plans consistently contain clear, detailed and person centred information to guide staff in delivering high quality care and support. The provider should also strengthen outcome focused planning to clearly identify what is important to people and how care and support will promote their health and wellbeing. This will support staff to understand how their practice contributes to achieving positive outcomes for individuals. This is to ensure care and support is consistent with the Health and Social Care Standards, which state that: • “I receive high quality care and support that is right for me.” (HSCS 1.1) • “My personal plan (sometimes known as a care plan) is right for me because it sets out how my needs will be met, as well as my wishes and choices” (HSCS 1.15).

Recommendation 2

1. The provider should further develop quality assurance systems to ensure there is a clear and consistent approach to monitoring and improving the quality of care and support. This should include the introduction of structured recording systems, such as central trackers, to provide an overview of key areas including reviews, actions taken and staff training. This will strengthen oversight, support accountability and provide clear evidence of how quality is monitored and improved. This is to ensure care and support is consistent with the Health and Social Care Standards, which state that: • “I benefit from a culture of continuous improvement, with the organisation having robust and transparent quality assurance processes” (HSCS 4.19).

Requirement 1

1. By 10 April 2026, the provider must ensure that the service is led well through the application of safe and accountable leadership practice to ensure people are protected and good outcomes are promoted. To achieve this, quality assurance systems must be improved take account of: A quality framework for support services (care at home, including supported living models of support) self-evaluation document, core assurance checklist and associated best practice guidance. This is to comply with Regulation 4(1)(a) of The Social Care and Social Work Improvement Scotland (Requirements for Care Services) Regulations 2011 (SSI2011/210).
