Requirement 1

1. By 22 November 2026, the provider must ensure there are robust governance arrangements in place to support the safe, consistent and effective delivery of care and support. To do this, the provider must, at a minimum: (a) Review and develop all policies and procedures required to support the operation of the service, including those relating to quality assurance and lone working. (b) Ensure policies accurately reflect current practice, legislation and good practice guidance. (c) Ensure staff are aware of and work in accordance with the service's policies and procedures. (d) Establish arrangements to regularly review and update policies to ensure they remain current and effective. This is to ensure that care and support is provided in a safe, consistent and well-governed manner and that people can be confident the service is operating effectively. This is in order to comply with: The Social Care and Social Work Improvement Scotland (Requirements for Care Services) Regulat2. By 22 November 2026, the provider must ensure that quality assurance processes are carried out effectively and in a manner which achieves improvements in the service. To do this the provider must ensure, at a minimum:

a) Routine and regular management audits are being completed across all areas of the service being provided. b) Internal quality assurance systems are able to identify any issue which may have a negative impact on the health and welfare of people supported, allow trends analysis and lessons learned. c) Clear action plans with timescales are devised where deficits and/or areas for improvement have been identified.

d) Action plans are regularly reviewed and signed off by an appropriate person in the organisation when improvements are achieved. This is to comply with Regulation 4(1)(a) (Welfare of users) of The Social Care and Social Work Improvement Scotland (Requirements for Care Services) Regulations 2011 (SSI 2011/210).

Recommendation 1

1. The provider should further develop and implement a comprehensive service improvement plan that clearly identifies improvement priorities, actions, timescales, responsible individuals and intended outcomes for people experiencing care. The plan should be regularly reviewed and evaluated to demonstrate progress, support continuous improvement and ensure leaders have effective oversight of the quality of the service. This is to ensure that care and support is consistent with Health and Social Care Standards (HSCS) which state that: ‘I benefit from a culture of continuous improvement, with the organisation having robust and transparent quality assurance processes'. (HSCS 4.19) and I use a service and organisation that are well led and managed (HSCS 4.23).

Recommendation 2

1. To ensure people are supported safely and well, the provider should develop and improve the quality of information within care plans. Care plans should be person centred, outcome focused and contain current and accurate updated information about people supported. This is to ensure that care and support is consistent with Health and Social Care Standards (HSCS) which state that: ‘My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met, as well as my wishes and choices (HSCS 1.15) and 'I am fully involved in developing and reviewing my personal plan, which is always available to me'. (HSCS 2.17).

2. To ensure people receive care and support that safely meets their needs, the provider should improve the quality and consistency of initial assessments. Initial assessments should contain sufficient, accurate and person-centred information about people's needs, preferences, outcomes and risks to support informed decision-making about the service's ability to meet people's needs safely and effectively. This is to ensure that care and support is consistent with Health and Social Care Standards (HSCS) which state that: ‘I experience high quality care and support that is right for me’ (HSCS 1) and ‘My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met, as well as my wishes and choices’. (HSCS 1.15)
