Recommendation 1

Recommendation (the Care Inspectorate's words)

1. The provider should strengthen quality assurance and governance arrangements to ensure audits and monitoring processes consistently identify and address recording omissions, medication management issues, and documentation gaps at the earliest opportunity. This should include robust oversight, clear action planning, and effective follow-up to evidence sustained improvement. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which states that: “I benefit from a culture of continuous improvement, with the organisation having robust and transparent quality assurance processes. (HSCS 4.19) and “As best practice develops, I benefit from new ways of working.” (HSCS 4.23)

Recommendation 2

Recommendation (the Care Inspectorate's words)

1. The provider should improve the consistency and quality of care planning documentation to ensure care plans, reviews, risk assessments, and daily records are fully up to date, accurately reflect current practice, and clearly evaluate progress towards personal outcomes. Documentation should provide a clear record of how support is improving people's wellbeing, independence, and quality of life. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which states that: My personal plan reflects my experiences and outcomes, as well as my wishes and priorities (HSCS 1.15) and I experience high-quality care and support because people have the necessary information and resources (HSCS 4.11)
