1. In order for people to be assured their care plan outcomes were up to date, six monthly reviews should be completed in full with effective detail. The provider should ensure that care plans clearly reflect any changes in people’s needs, circumstances, and outcomes, and that all plans are updated to show each person’s current position. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'My care and support meets my needs and is right for me' (HSCS 1.19); and ‘My personal plan is right for me because it sets out how my needs will be met, as well as my wishes and choices’ (HSCS 1.15).
2. In order for people to benefit more from accessing their community and other social activities in reliable and adapted transport, the provider should ensure the vehicle is kept in good working order, with no long gaps when it is out of commission. This will help people maintain community connections and participate in activities that are important to them. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'I am supported to participate fully and actively in my community' (HSCS 1.10); and ' My care and support meets my needs and is right for me' (HSCS 1.19).
