Positive Paths Children and Young People's Service

1. To ensure young people's needs are effectively met, the service ensure care plans are kept up to date. This should include but is not limited to, reviewing processes and responsibilities for care planning updates within the service. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state: “My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met a well as my wishes and choices” (HSCS, 1.15).
