1. By 26 June 2026, the provider must ensure people receive the care and support that is right for them, and their thoughts and wishes are respected and recorded within their care and support plan. In order to achieve this the provider must: a) Ensure accurate recording of all care and support provided by staff, to facilitate effective evaluation. b) Ensure accurate recording of all accidents, incidents and adverse events within each persons written plan. c) Ensure the standard and quality of record keeping should be consistent with the provider's policy, as well as Nursing & Midwifery Council (NMC) and Scottish Social Services Council (SSSC) codes of practice. d) Ensure that the written plan, any and all associated documentation is being effectively monitored and audited. This is to ensure care and support is consistent with Health and Social Care Standard 1.15: My personal plan (sometimes referred to as a care plan) is right for me1. The provider should implement the time limited recovery plan submitted to the care inspectorate on 15 April 2026. In particular, you must ensure that: a) An updated copy of the time limited recovery plan is submitted to the care inspectorate every 2 weeks. b) Staff responsible for clinical oversight have the necessary skills and knowledge to assess service users’ health, safety and wellbeing needs, including when there is a significant change in those needs. c) Service users receive assistance with their care needs at times that meet their needs and preferences. d) Nursing and care staff understand and fulfil their roles and responsibilities in relation to promptly identifying, reporting and responding when there are changes in service users’ health, wellbeing or safety needs, including when s
1. Ensure every concern, complaint, incident, accident and allegation is thoroughly investigated within defined timescales, with written reports detailing findings, corrective actions and lessons learned to improve individual outcomes.
1. To support people's health and wellbeing the provider should ensure all adverse incidents or accidents, unexplained injuries and bruising are fully investigated, and appropriate action taken to reduce harm to people. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which states that: 'My care and support meet my needs and is right for me' (HSCS 1:19); and 'My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met, as well as my wishes and choices' (HSCS 1.15).1.
