Recommendation 1

1. To improve the quality of information for staff, the provider should ensure that personal plans and risk assessments are detailed and reviewed regularly. Personal plans should contain sufficient information on what care people need and how to offer this in a way that meets their needs and wishes. They should be reviewed on a six-monthly basis or more frequently as people's needs change. This is to ensure that care and support is consistent with the Health and Social Care S tandards (HSCS) w hich s tate that: 'My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met as well as my wishes and choices' (2. To ensure people receive the safe support with medication, the provider should ensure that people receive their medication at the right time and that this is accurately recorded. This should include, but not be limited to, completing regular audits of medication administration records and visit times. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'Any treatment or intervention that I experience is safe and effective' (HSCS 1.24).
