1. By 24 August 2026, the provider must ensure robust and effective governance and quality assurance systems are in place to monitor the quality and safety of the service. To achieve this, the provider must, at a minimum: a) Implement a structured programme of audits and ensure findings are analysed and used to drive improvement. b) Develop and maintain an overarching service improvement plan informed by audits, incidents, review processes and feedback from people. c) Ensure this is aligned with the records a service is required to keep with care reviews, incidents, and service data consistently used to inform learning, drive improvement, and meet statutory responsibilities.
