1. By 31 May 2026, the provider must ensure that young people are supported in a service that ensures their safety and wellbeing through robust quality assurance processes. To do this, the provider must, at a minimum: a) ensure effective quality assurance systems are implemented to provide scrutiny, analysis and assurance that care is safe, well planned, documented and regularly reviewed; b) undertake a review of all young people's personal plans. These should include the contributions of young people and the multi-agency team around the child. To ensure that young people receive appropriate care and support, personal plans should outline clearly a young person’s support needs, the desired outcomes for the young person and the role of the service in meeting these. This is to ensure that the quality of care is consistent with the Health and Social Care Standards (HSCS) which state that: 'My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met, as well as my wishes and choices.' (HSCS 1.15).
2. To ensure that young people have the right care and support to help them successfully transition into adulthood, the provider should develop greater awareness of their duties under Continuing Care legislation, including the completion of welfare assessments This is to ensure that the quality of care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'My future care and support needs are anticipated as part of my assessment.' (HSCS 1:14).
