Conditions of registration
Any other conditions unique to the service:
1. To provide a service to adults with learning/physical disabilities and people with mental health support needs and/or drug and alcohol dependant living in their own homes.
2. The support will be provided by four staff teams as agreed during this registration and where there are any subsequent changes the Care Inspectorate should be informed.
3. The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details, as it appears on this certificate.
