Recommendation 1

1. The provider should ensure that they keep people safe and healthy by ensuring that all accidents and incidents are properly managed. To do this, the provider should: a) Have a system in place to regularly monitor, review, and learn from accidents and incidents. b) Ensure that notifications are made to the Care Inspectorate in accordance with Care Inspectorate guidance 'Adult care services: Guidance on records you must keep and notifications you must make' (March 2025). This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: ' I benefit from a culture of continuous improvement, with the organisation having robust and transparent quality assurance processes' (HSCS 4.19).

Recommendation 2

1. To improve the quality of information for staff, the provider should ensure that support plans are clear, sufficiently detailed, and person-centred. This should include, but not be limited to: a) Ensuring legal powers are detailed and outlined within personal plans with information about staff responsibilities. b) Plans are in a format that is accessible and meaningful to each person. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'My personal plan (sometimes referred to as a care plan) is right for me
